I am releasing my pet to Elm Point Animal Hospital for boarding. I understand that the doctor’s and staff will take reasonable precautions to prevent injury, escape, or death of my pet, but I do understand that they will not be liable for circumstances beyond their control. I also understand that Elm Point Animal Hospital is not responsible for any items such as blankets or toys that may become lost or damaged during boarding. Collars and leashes will not be kept. I will be charged for the day I drop off my pet through and including the day I pick up my pet, unless I arrive before 12 noon.
